1. Open your new Hyde Park Bank checking account.
Complete and send to Hyde Park Bank, 1525 East 53rd Street, Chicago, Illinois 60615:
  New Account Information Form
  Visit either location of Hyde Park Bank within 30 days to fund your account, sign your signature card and present photo identification and proof of residency.  Your Personal Banker will provide your account number and account documents, assist you in ordering checks and answer any questions you may have.

 

 

2. Transfer automatic deposits and payments.
Complete and send to your employer’s payroll department, or other payer:

3. Close your old account.
